I wish to apply for the Membership of PIOCCI. I am desiring to become a Life/Ordinary member of your valued institution. I am submitting the required particulars hereunder and also enclosing my KYC* documents and the payment. I undertake that I will act in accordance with the Memorandum of Association and the Rules and Regulations and also the bye-laws which might be adopted by the Governing Council and the General Body of the PIOCCI from time to time.

I also undertake that I am eligible to become a member of the PIOCCI and understand that if at any time, due to any reason, I may become debarred from the membership under the Rules and Regulations, then such debarment will be accepted by me gracefully.
